Person Centred Planning

What is it?

Person Centred Planning (PCP) is a way of working alongside a person who wants to make changes in their life.

This involves bringing together all the people who are important to the person, to listen and learn about them. A commitment is given to help them gain a more meaningful and fulfilling life.

PCP is a way of finding out how a person wants to live, where they want their life to go and what needs to be done to get there.

PCP assumes that all people have hopes and dreams and that their aspirations will change and grow with experience.

Who is PCP for?

Anyone may use and benefit from PCP. There are no age limits.

A person who has a specific need for support or anyone facing significant life change may find this process especially helpful. It can also be useful to teams, groups, communities, businesses and organisations.

What does PCP do?

PCP provides many benefits:

  • An opportunity for the person and their family to say what they want or need to improve their quality of life.
  • An individual plan of action
  • New opportunities
  • The chance to develop new skills and abilities
  • More individual choice
  • Support within the lifestyle the person wants
  • Adapt services to reflect the person's desires and needs
  • Connecting a person to their community

There are a number of ways to plan, depending on what the individual wishes to plan for. The main approaches are:

  • Personal Futures Planning
  • Maps
  • Paths
